Do you offer female annual exams?

A.

Yes! We do annual pap smears and well woman exams. Any referrals for Mammograms, DEXA Scans, or Blood Testing will be sent to the Radiology, or Lab vendor of your choice.

Do I need to tell you about my past medical history?

A.

Yes, in order to provide you the best care possible it is essential that we know as much as possible about your past medical history including medications, visits to specialist, health status, recent test results, self-care information, recent hospitalizations, and specialty care or ER visits.

What aesthetic procedures do you offer?

A.

We offer a range of toxin and fillers, chemical peels, facials, and Platelet Rich Plasma.

What should I do if I have an urgent medical need?

A.

If you have a life-threatening emergency, immediately call 911 or go to the nearest hospital. Notify your health plan and call your doctor’s office as soon as possible. If your urgent medical need is related to your treatment by Angela Lorenzo PA-C at Modern Medical and Wellness, and occurs during our office hours, call our office directly. If your urgent medical need occurs after office hours, simply call our office and leave a voicemail, and go to the nearest hospital. Your phone call will be returned within 24-72 hours.

How do I get my prescriptions refilled?

A.

We recommend that you schedule your follow up appointment with your physician prior to running out of your medication.

Can my spouse or relative pick up my records?

A.

Yes, as long as they are specified on your release of medical information form.

Can my Spouse speak to you about my account?

A.

Due to the Health Information Portability and Accountability Act (HIPAA) we cannot disclose patient information to anyone, regardless of relationship, other than the patient without their consent. You may fill out a release of information form that will allow the doctor's office to speak to the person you designate. Please see our Patient Privacy Notice for more information. There is also an option to create a "Family Account" on our patient portal.

Is there a charge to receive my records?

A.

The charge to receive medical records is $0.60 per page.

How long does it take to get my records?

A.

A medical record request may take up to thirty days to process. Records will be released after the applicable payment.  Under certain special circumstances that time can be shortened.

Will I be able to get copies of my test results?

A.

You may get copies of you lab results upon request.

How do I obtain my medical records?

A.

You must call the office or walk in to fill out a Medical Record Request. This request will then be submitted to the medical record administrator and will be processed within 30 business days.

Are my records secure and confidential?

A.

Yes, we make every effort to ensure your privacy and confidentiality in all facets of serving you. Please review the “Patient Privacy” information on our website if you have questions.

How will I be reminded that I have an appointment?

A.

You will receive multiple reminders in the form of a text message as well as email. We do ask that you give us a call to confirm your appointment and if possible confirm the reason for the appointment also, i.e. medical issue, diet, etc. the importance of this is each reason to be seen by the doctor has time standards attached to it, if you come in for one reason and the appointment has to be extended for a separate diagnostic reason additional charges will be added for the extended time and diagnostic exam(s).

What is your cancellation policy?

A.

In order to provide better service to our patients, we do not overbook to compensate for no shows and late cancellations. Should you need to change or cancel an appointment or procedure for Modern Medical & Wellness, please contact us 48 hours before your scheduled appointment. Any cancellations with less than 48hours notice will result in full charges billed to your credit card on file.

What if I am late for my appointment?

A.

If you’re late for your appointment you will be seen  for  the remainder of your appointment time in order to avoid delays. If you are more than fifteen minutes late for your appointment, you will be charged for the additional time spent with the doctor. Please be sure to review our office policies.

Do you allow urgent or same day appointments?

A.

Yes, just call the office and let the receptionist know that you need an appointment. We are usually fully booked, but it is always good to call and ask if there is any last minute opening.

How early should I arrive for my scheduled appointment?

A.

We ask you to arrive 15-minutes prior to your scheduled appointment time. This allows ample time for check-in and the completion of any necessary paperwork. This is also a courtesy to the patients who have appointments following yours. We want everyone to get their fair share of time with the Doctor.

How can I get answers to questions about the bill I received from my doctor?

You can call the Billing hotline number anytime. The phone number is 1-855-245-5055. This number is also located on your statement.

What forms of payments do you accept?

A.

For your convenience, our practice accepts cash, personal checks, debit, and credit cards (Visa, MasterCard, Discover, and American Express). Unfortunately, there is a small fee for any debit or credit card transactions.

Can you waive my co-pay or other fees if I don’t have the money?

A.

As your doctor, we are obligated by law to apply your benefits according to your health plan’s guidelines. We cannot waive co-pays. We can, however, make payment arrangements with you.  Call our Office to discuss your options.

How much am I expected to pay?

A.

Your copay and deductible and any non contracted usual and customary fees. If you do not have insurance you are expected to pay the cash pay price in full at time of service.

What insurances are you in network with?

A.

Modern Medical and Wellness accepts a variety of health plans. Please call our office to ask about insurances accepted.  If we do not already contract with your health plan or insurance company, we may still be able to obtain an authorization to see you on an out-of-network basis.

Tell me more about Doctor Carmen F. Jones.

A.

Dr. Jones relocated to Las Vegas in 2002. She completed her undergraduate and medical degrees from the University of Illinois. She completed her pediatric residency training at Rush-Presbyterian St. Luke’s Medical Center in Chicago. Her experience is widely varied. She has worked with the underserved, in both the inner city and in rural areas, and with children placed in the Cook County foster care system, in Chicago. She has served as a hospitalist working with neonatal and pediatric emergencies. Now after 24 years in medicine, she has chosen to diversified her practice. So, in addition to pediatric practice with Foundation for Positively Kids, that serves the foster care children in Clark County, NV, she is also the Medical Director and primary care physician for the Modern Medical & Wellness Clinic and a Physician Consultant for the Health and Wellness Center. Dr. Jones currently serves as the Executive Director of HerShe Las Vegas, Inc. This is a nonprofit organization the assists young women in their transition from foster care into adulthood. She is the former Treasurer of the Las Vegas-Clark County Urban League Young Professionals. She has served as a board member of 20 Pearls Foundation, Inc, of Las Vegas, and is an active member of Alpha Kappa Alpha Sorority, Inc. She is very close to her family and friends, loves to travel, enjoys the arts, and attempts to speak Spanish and French whenever possible.

Tell me more about Ms. Angela Lorenzo PA-C.

A.

Angela Lorenzo PA-C practices Internal Medicine and Medical Weight Loss. Her heart and expertise is in Aesthetics, Hormones, and Medical Weight Loss. She believes it is the core to your wellness. She will treat the patient, not just the test results. Angie has been a PA-C for 11+ years. Prior to that she was an EMT and a Nurse. She attends several conferences yearly to be on the cutting edge of today’s medical advances. She was the first in Las Vegas to practice Bio-Identical Hormone Pellet Implantation and she trained a large majority of the medical practitioners here in the LV valley. She also has a passion for sexual wellness and sexual dysfunction. She was certified as a Sexual Health Practitioner with the American Academy of Anti-Aging Medicine in2013. She is one for two medical providers that are certified in this area of medicine in Nevada. She also is active with the AAPA, AMA, NAPA, AA4M organizations. She chairs the Las Vegas Academy of Behavioral Health and coordinates the Medical Supervision of several Las Vegas Behavioral Centers, and works with the charity iHumanity to help children with behavioral needs. Angie Lorenzo invented a Burn Cream that is patented since2014. She is planning on licensing this so it’s available to everyone! It can be used for sunburns, burns, razor burn, rashes, bug bites, and dry skin. (This is now a prescription when issued by a licensed medical provider.)
